PROGRAM DESCRIPTION:
This course is done under the SPSC-WHOcc activities in alignment with the third WHO global patient safety challenge. One of the most critical activities that healthcare providers can perform to improve the safety and quality during transition of care is medication reconciliation. This program is a training framework that help participant to gain knowledge on the subject and be able to train and teach others. It summarizes the essential steps to conduct medication reconciliation in a healthcare setting utilizing the WHO-High5 -standard operating protocol and international Patient Safety and quality standards.
Objectives:
- To describe the importance of Medication Reconciliation process.
- To present the principles and strategies to spread and measure the improvements in Medication Reconciliation.
- To recognize the Physician, pharmacist, and Nurses' role in this process.

• Physician
• Nurse
Outcomes:
- Outline the key steps for effective and safe Medication Reconciliation.
- Given Patient scenario, accurately identify the appropriate resources and skills needed to
complete medication reconciliation.
- Developing medication reconciliation competency program for health care professionals
- Sharing challenges and lessons learned.
PRE-/POST-TEST Guidance:
- Each participant will conduct the test twice before and after the training session.
- The main objective is to assess the training effectiveness.

- The test contains 5 questions, each correct answer has score of 20%, and the total full mark is 100%.

A passing score is 60% or more is required to issue the certification.
